What is wrong with my fantail goldfish?

He just lays upside down at the bottom of his tank all day... He's been doing this for over a month. Is this just swim bladder disease? Several times I've tried to feed him a chopped pea (out of the husk) and he eats it, but he doesn't change. He also has had white stuff kind of clinging to him. On his tail there is one white circle, it is a perfect circle and it's larger than an ick spot, plus there's only two or three and it doesn't look like he's been sprinkled in salt. He also has one spot on his head, not on his face but it's above his eyes, underneath his dorsal fin. This spots leave and then reappear in around the same area. What's wrong with him?
